Initial Appointment: 

• 1 hour consultation with Dr Michael Hatzifotis
• $250 fee (Medicare rebate: $76.15)
• Valid GP referral required
• BMI of 35+ or 30+ with co-morbidities
• Completed patient information form and signed privacy consent
• Any relevant test results
• The Medicare rebate can be transmitted from our clinic on the day of your appointment
